RICCC
This is the main page of the RICCC, there are 4 other pages 3 which are linked at the top of this page and one that is a secure page only accessible by team members.
The RICCC is a instate collaborative effort to assist providers in looking at a different way to manage patients with chronic diseases. The focus of the collaborative is Diabetes, but all of the training can be transferred to any other chronic condition over time as well as to managing preventative medicine issues (Paps, Mammograms, Immunizations, etc).
The next collaborative will be starting in the Spring of 2006. We are currently recruiting teams and would like to hear from anyone that is interested in joining the collaborative effort or have questions.
